New Patient Registration
To register with the practice you need to fill our registration forms. You will be offered a new patient appointment with a practice nurse if this is appropriate, or if you take regular medication you will need to see a doctor before you need to order it.
Please print off and complete an NHS Scotland registration form and the appropriate new patient questionnaire from the links below. Alternatively, a paper copy is available at the practice reception (one registration form and one questionnaire for every person registering).
Once completed, please bring it in to the medical centre along with relevant identification and we can register you as a patient with this practice.

The practice area covers the Trotternish peninsula, extending as far south as Sconser and as far west as Kildonan. We also provide medical services on the Island of Raasay.
If you stay outwith our catchment area you will need to attend an alternative practice. There are other local practices in Dunvegan, Carbost, Broadford, Sleat and Kyle of Lochalsh.
