New Patient Registration
To register with the practice, please complete our registration forms. These forms can be completed electronically or by hand. Each patient is required to complete the NHS Scotland registration form and the appropriate health questionnaire for patients - these are linked below but can also be collected at reception. A signature is required on both forms.
Completed forms can either be handed in or emailed to us. We require to see identification when registering.
New patients will be offered an appointment with a practice nurse if this is appropriate. If a new patient takes regular medication they will need to see a doctor before this can be ordered.
